IPOH - The Department of Occupational Safety and Health (DOSH) has issued a directive to halt cleaning operations at a cement factory in Chemor, situated nearby, following a tragic death of a worker on Monday.

The incident, which occurred around 3.15pm, resulted in the death of a Bangladeshi worker identified as Roki Md Jadidur Rahman, 26, due to being crushed by cement.

Perak DOSH Director Dr Rabaayah Daud said the investigation revealed that during the incident, the victim and two co-workers were breaking up hardened cement inside the maintenance hole of the silo using a hammer and iron rod.

During the work, the cement inside the silo spewed out through the maintenance hole, causing the victim to fall and be buried under the cement.

"The victim was taken to Sungai Siput Hospital for treatment but was pronounced dead by medical officers.

"Following the initial investigation, DOSH issued a prohibition notice to the employer to cease cleaning work on the silo until the employer takes corrective actions," she said in a statement here on Wednesday.

She said that legal action would be taken against those responsible if there were violations under the Occupational Safety and Health Act 1994.

She added that such incidents could be avoided if employers carry out a comprehensive hazard assessment and risk control according to the methods outlined in the Guidelines for Hazard Identification, Risk Assessment, and Risk Control (HIRARC).

"In this case, employers are responsible for identifying workplace hazards, conducting risk analysis, and implementing adequate risk controls before workers engage in activities.
